Use this form as soon as you notice an issue with any equipment or vehicle. Flag the equipment with ribbon near the damage, describe the issue below, and click the button to email your shop manager. Don't forget to attach a photo!

After clicking the button below, your default email app will open. Be sure to add the correct recipient(s) and attach a photo of the issue before sending. CC your Crew Leader, Supervisor, or others affected.

  • 📤 When you receive a repair ticket: forward it to any other team members who may need to know.
  • đź“… Provide a quick update: reply with status like “Received – part ordered” and include an estimated repair completion date.
  • âś… Mark it complete: once repaired, reply “Back in service” and archive it or move it to a folder named Completed Repair Tickets.
  • 📌 Flag or pin the email: keep unresolved repairs visible at the top of your inbox until finished.
  • âť“ Ask for clarification: if anything’s unclear, respond and confirm before action is taken.

🔴🟡Tool Tagging Protocol – Ribbon & Report System

Here’s how it works:
No ribbon means go. That’s the standard—fueled, functional, and field-ready. If a tool can’t meet that by the end of the day, it gets a yellow ribbon. Yellow means something’s off or needs attention. Attach the ribbon and complete the Equipment Repair Request Form—and be sure to copy your manager, the equipment supervisor, and any team that might use it next.

If a tool is unsafe or out of commission, it gets a red ribbon. That means it’s officially down and waiting for shop repair. Red-tagged tools stay parked until cleared.

Keep rolls of yellow and red ribbon (flagging tape) in every truck and in the employee room.
It’s simple. It’s visual. And it keeps everyone in the loop.
